Recent advances in systemic lupus erythematosus and microbiota: from bench to bedside

摘要: Systemic lupus erythematosus (SLE) is a complicated autoimmune disease affecting multiple systems and organs. It is highly heterogeneous, and it preferentially affects women at childbearing age, causing worldwide social burden. The pathogenesis of SLE mostly involves genetic predisposition, epigenetic dysregulation, overactivation of the immune system, and environment factors. Human microbiome, which is mostly composed of microbiota colonized in the gut, skin, and oral cavity, provides a natural microbiome barrier against environmental risks. The past decade of research has demonstrated a strong association between microbiota and metabolic diseases or gastrointestinal diseases. However, the role of microbiota in autoimmunity remains largely unknown until recently, when the technological and methodological progress facilitates further microbiota research in SLE. In this review, the latest research about the role and mechanisms of microbiota in SLE and the advances in the development of diagnostic and therapeutic strategies based on microbiota for SLE were summarized.

Control of lupus activity during pregnancy via the engagement of IgG sialylation: novel crosstalk between

摘要: Immunoglobulin (IgG) glycosylation affects the effector functions of IgG in a myriad of biological processes and has been closely associated with numerous autoimmune diseases, including systemic lupus erythematosus (SLE), thus underlining the pathogenic role of glycosylation aberration in autoimmunity. This study aims to explore the relationship between IgG sialylation patterns and lupus pregnancy. Relative to that in serum samples from the control cohort, IgG sialylation level was aberrantly downregulated in serum samples from the SLE cohort at four stages (from preconception to the third trimester of pregnancy) and was significantly associated with lupus activity and fetal loss during lupus pregnancy. The type I interferon signature of pregnant patients with SLE was negatively correlated with the level of IgG sialylation. The lack of sialylation dampened the ability of IgG to suppress the functions of plasmacytoid dendritic cells (pDCs). RNA-seq analysis further revealed that the expression of genes associated with the spleen tyrosine kinase (SYK) signaling pathway significantly differed between IgG- and deSia-IgG-treated pDCs. This finding was confirmed by the attenuation of the ability to phosphorylate SYK and BLNK in deSia-IgG. Finally, the coculture of pDCs isolated from pregnant patients with SLE with IgG/deSia-IgG demonstrated the sialylation-dependent anti-inflammatory function of IgG. Our findings suggested that IgG influences lupus activity through regulating pDCs function via the modulation of the SYK pathway in a sialic acid-dependent manner.

Mycophenolate mofetil or tacrolimus compared with azathioprine in long-term maintenance treatment for active lupus

摘要: This study aimed to evaluate the efficacy and safety of mycophenolate mofetil (MMF) or tacrolimus (TAC) compared with azathioprine (AZA) as maintenance therapy for active lupus nephritis (ALN). Patients with ALN who responded to 24 weeks of induction treatment were enrolled. Patients who received MMF or TAC as induction therapy continued MMF or TAC treatment during the maintenance period, whereas those who received intravenous cyclophosphamide were subjected to AZA treatment. The primary endpoint was the incidence of renal relapse. Secondary endpoints included extrarenal flares and composite endpoints (deaths, end-stage renal disease, or doubling of serum creatinine levels). A total of 123 ALN patients (47 in the MMF group, 37 in the TAC group, and 39 in the AZA group) were enrolled. The median follow-up time was 60 months. Ten MMF-treated patients, ten TAC-treated patients, and eight AZA-treated patients experienced renal relapses (P = 0.844). The cumulative renal relapse rates in the MMF group (P = 0.934) and TAC group (P = 0.673) were similar to the renal relapse rate in the AZA group. No significant difference in the incidence of severe adverse event was observed among the groups. Long-term maintenance therapies with MMF or TAC might have similarly low rates of renal relapse and similar safety profiles compared with AZA.

A systemic review of hydrogen supply chain in energy transition

摘要: Targeting the net-zero emission (NZE) by 2050, the hydrogen industry is drastically developing in recent years. However, the technologies of hydrogen upstream production, midstream transportation and storage, and downstream utilization are facing obstacles. In this paper, the development of hydrogen industry from the production, transportation and storage, and sustainable economic development perspectives were reviewed. The current challenges and future outlooks were summarized consequently. In the upstream, blue hydrogen is dominating the current hydrogen supply, and an implementation of carbon capture and sequestration (CCS) can raise its cost by 30%. To achieve an economic feasibility, green hydrogen needs to reduce its cost by 75% to approximately 2 /kg at the large scale. The research progress in the midterm sector is still in a preliminary stage, where experimental and theoretical investigations need to be conducted in addressing the impact of embrittlement, contamination, and flammability so that they could provide a solid support for material selection and large-scale feasibility studies. In the downstream utilization, blue hydrogen will be used in producing value-added chemicals in the short-term. Over the long-term, green hydrogen will dominate the market owing to its high energy intensity and zero carbon intensity which provides a promising option for energy storage. Technologies in the hydrogen industry require a comprehensive understanding of their economic and environmental benefits over the whole life cycle in supporting operators and policymakers.

Progress in systemic therapy for triple-negative breast cancer

摘要: Triple-negative breast cancer (TNBC) is the most aggressive subtype of breast cancer with a heterogeneous genetic profile. Chemotherapy exhibits substantial activity in a small subset of these patients. Drug resistance is inevitable. Major progress has been made in the genetic analysis of TNBC to identify novel targets and increase the precision of therapeutic intervention. Such progress has translated into major advances in treatment strategies, including modified chemotherapy approaches, immune checkpoint inhibitors, and targeted therapeutic drugs. All of these strategies have been evaluated in clinical trials. Nevertheless, patient selection remains a considerable challenge in clinical practice.

Distinct roles of ASIC3 and TRPV1 receptors in electroacupuncture-induced segmental and systemic analgesia

Previous studies have demonstrated the effects of different afferent fibers on electroacupuncture (EA)-induced analgesia. However, contributions of functional receptors expressed on afferent fibers to the EA analgesia remain unclear. This study investigates the roles of acid-sensing ion channel 3 (ASIC3) and transient receptor potential vanilloid 1 (TRPV1) receptors in EA-induced segmental and systemic analgesia. Effects of EA at acupoint ST36 with different intensities on the C-fiber reflex and mechanical and thermal pain thresholds were measured among the ASIC3/, TRPV1/, and C57BL/6 mice. Compared with C57BL/6 mice, the ipsilateral inhibition of EA with 0.8 C-fiber threshold (0.8Tc) intensity on C-fiber reflex was markedly reduced in ASIC3/ mice, whereas the bilateral inhibition of 1.0 and 2.0Tc EA was significantly decreased in TRPV1/ mice. The segmental increase in pain thresholds induced by 0.3 mA EA was significantly reduced in ASIC3/ mice, whereas the systemic enhancement of 1.0 mA EA was markedly decreased in TRPV1/ mice. Thus, segmental analgesia of EA with lower intensity is partially mediated by ASIC3 receptor on Aβ-fiber, whereas systemic analgesia induced by EA with higher intensity is more likely induced by TRPV1 receptor on Ad- and C-fibers.

人与地球系统之间的相互作用已成为全球变化的重要驱动力,极地系统是感应全球变化的放大器和指示器极地系统科学进入了一个大科学研究时代。极地系统科学不但是一门研究极地系统对全球变化的响应与反馈作用中运转的机制、规律和控制这些变化的机理,并为区域复杂性及其对全球变化预测提供科学基础的科学,同时也是一门高新技术应用和人类认识自然活动并科学管理极地系统的系统工程学。20年来,中国科学家对极地系统复杂性有了深入的认识,对极地科学考察管理也进行了有益的探索。

Sclerosing cholangitis in critically ill patients: an important and easily ignored problem based on a German experience

Intensive care unit (ICU) is important in the rehabilitation of critically ill patients. In the past decades, many patients who received aggressive treatment in ICU developed sclerosing cholangitis in multiple centers. Sclerosing cholangitis in critically ill patients (SC-CIP) is a relatively new issue. To investigate the causes, clinical manifestation, treatment, and prognosis of SC-CIP, we searched for published cases in the databases of PubMed, Highwire, and Elsevier from 2001 to 2012. Data were extracted using a standard form and retrospectively analyzed. Twelve eligible studies covering 88 patients, with 64 men and 24 women, were enrolled in this analysis. The mean age was 49.8 years. All of the patients recovered from critical illnesses, such as trauma, infection, burn, and major surgeries. High pressure positive end-expiratory pressure (PEEP, peak level at 12.8 cm H2O) was utilized for all patients, with the average duration of 36.3 d. In addition, vasopressor agents were administered in approximately 60% of SC-CIP. A rapid increase in cholestasis and irregular strictures in the intrahepatic bile ducts was observed in the following months. With an average follow-up period of 17.9 months, poor outcomes were observed in 54 patients, including 34 deaths. In conclusion, ischemic injury of the biliary tree, which may be affected by PEEP and/or vasopressor administration, affects cholangiopathic procedure. As a newly discovered type of secondary sclerosing cholangitis, SC-CIP is a severe progressive complication of patients in ICU and should be carefully monitored by clinicians.

金黄色葡萄球菌(S. aureus)是一种常见的人类病原体,它可以引发严重的侵袭性感染,如菌血症、败血症和心内膜炎,具有较高的发病率和死亡率。然而由于细菌的抗生素耐药性增强,如耐甲氧西林金黄色葡萄球菌(MRSA),加剧了此类细菌的发病率和死亡率。金黄色葡萄球菌的发病机制是由毒素的分泌推动的,如膜损伤孔α毒素,它有不同的细胞靶点,包括上皮细胞、内皮细胞、白细胞和血小板。本文采用人体血小板膜包覆纳米颗粒(PNP)作为一种仿生诱饵策略,来中和金黄色葡萄球菌的毒素,并维持宿主细胞的防御功能。血小板膜包覆纳米颗粒保护血小板免受由金黄色葡萄球菌毒素带来的损伤,维持血小板活化和杀菌活性。血小板膜包覆纳米颗粒也同样保护巨噬细胞免受由金黄色葡萄球菌毒素带来的损伤,支持巨噬细胞进行氧化迸发、产生一氧化氮和维持其杀菌活性,并减少耐甲氧西林金黄色葡萄球菌诱导的中性粒细胞胞外杀菌网络。在感染系统性耐甲氧西林金黄色葡萄球菌的小鼠模型中,血小板膜包覆纳米颗粒制剂减少了血液中的细菌数量并防止小鼠发生死亡。总之,目前的研究结果证明了血小板膜包覆纳米颗粒的治疗优点,如中和毒素、保护细胞和增加宿主对侵袭性金黄色葡萄球菌感染的抵抗力。
